Importance of Care

After hospital discharge, all your concerns and baby’s concerns will be followed by your family doctor or nurse practitioner. These routine visits with your care provider are important to check for problems and make sure that your baby is growing and developing as expected. Your family doctor or nurse practitioner can provide ongoing, comprehensive care for you and your baby.  Don’t be afraid to call your care provider any time you have concerns about your baby’s health.

Need a family doctor or nurse practitioner? Please visit the Attachment Hub to register for the waitlist for a family doctor or nurse practitioner.

If you are waiting for attachment to a family doctor or nurse practitioner and need care in the meantime, please visit the Pathways Medical Care Directory.
